**Important Notice regarding Humana Preferred.  As of  8/1/08 we will no longer be participating in network with a specific version of Humana.  If your insurnace card says "Humana Preferred PPO" or "Humana Preferred HMO or POS" in the upper left corner, we will not be in network.
